Nutrition Assessments - Facility Visit

Service Description

This package is offered to facilities only. It includes a facility visit for initial nutrition assessment and follow-ups for up to 4 clients per visit.

Cancellation Policy

To cancel or reschedule, please contact us at least seventy two (72) hours in advance. 1. Cancellation: 1.1 Cancellations with at least seventy two (72) hours notice are subjects to a five ($5.00CAD) dollar processing fee. 1.2 Cancellations within seventy two (72) hours will be subject to the full cost of the appointment. 2. Rescheduling: 2.1 Rescheduling with at least seventy two (72) hours notice will be free of charge for the changes 3. No shows 3.1 No shows without notice will be subject to the full cost of the appointment.
